All clients that have health and/or dental coverage through USA Payroll must complete an Employee Health & Dental Deduction Form in order to change deductions amounts for employees. This form must be completed and returned to your Payroll Specialist prior to your first payroll run after the deduction change effective date in order to ensure accuracy. Also, please note that when terminating health and/or dental coverage for any employees, USA Payroll must have something in writing to process the request.
